YOUR RESPONSIBILITY AS A MTABF MEMBER

It is your responsibility to inform the MTABF in writing of a CHANGE OF ADDRESS or CHANGE OF STATUS (birth, death, divorce, legal separation, a child losing dependent status, retirement, etc.) within 60 days of the date of the event.

You can Update your MTABF information by filling out a CHANGE OF STATUS FORM.
