YOU MUST notify the MTABF of any life changes that will impact YOUR BENEFITS.
Some life events that will impact your benefits are:
IF YOU EXPERIENCE A LIFE CHANGE:
- Baby - include copy of birth certificate
- Death of dependent / spouse
- Divorce - include copy of agreement -signature page. Don't forget to update your life insurance beneficiary.
- Leave of Absence - coverage may lapse
- Married - include copy of marriage certificate. Don't forget to update your life insurance beneficiary.
- Move - change of address
- Retiring - go here
- ANY OTHER CHANGES in YOUR LIFE that will impact YOUR BENEFITS
IF YOU EXPERIENCE A LIFE CHANGE:
- Complete the entire form(s).
- Include any necessary documentation (marriage certificate, birth certificate, etc.)
- Give all paperwork to your building trustee or mail to the PO Box on on the enrollment form.
- MTABF will then submit the info to ASO. You may also upload documents through the ASO Member Portal.
